If you are new to officiating or have officiated for less than 2 years, please review, complete, and submit the following:

DBOA Enrollment Application

Used to enroll in DBOA and UIL. Must be completed annually.

Required for all DBOA officials to complete on an annual basis.

Transferring Officials


If you have officiated for 2 years or more and are transferring to the DBOA from another chapter, please complete and submit the following:

Forms and Reports


This section includes forms and reports needed for officials to carry out chapter business.

Documents
Description

School Report Form

Used to report infractions of UIL rules pertaining to hospitality and coaching box. (Online form coming soon.)

IRS Form W-9

Must be completed annually for each school district or private school.

DBOA Enrollment Application

Used to enroll in DBOA and UIL. Must be completed annually.

Basketball Point Recording Form

All transferring and existing officials are required to complete and submit annually with enrollment application. First-year officials are exempt.

Physical Examination Form

Required for officials new to DBOA. (TASO)

Professional Responsibility Contract

Required for all DBOA officials to complete on an annual basis.

DISD P1C Form

Officials who work in DISD are required to submit this form at every assignment.

DBOA Scrimmage Report

Lead officials are to fill this in and submit it after all scheduled scrimmages.

DBOA Scrimmage Instructions for Lead Officials

Lead officials, please read this so you will know what is required of you during and after the scrimmages.

DBOA Expense Form (PDF) (Excel)

Used to request reimbursement for expenses occurred while conducting official DBOA business.

Late or Missed Assignment Report

Must be completed and submitted to Assignment Secretary within 24 hours of late or missed assignment.

Insurance Claim Form

To file a claim for accidental injuries, print and complete in its entirety. See NFHS Insurance Program (above) for more details.
